As a follow up to last week’s topic, The Legal Documents You Need When Your Child Turns 18, a parent has asked, “Could you also post how to get those forms and documents prepared? I’ve been trying to get those done but found it very difficult to find and nowhere to notarize it.”
Resources for obtaining the forms weren’t listed in the article because the forms are specific to each state, as determined by your state of residence. “Do it yourself” versions are available by searching online by entering “medical and/or durable power of attorney”, and “your state” for (often) free, downloadable forms, as well as what the requirements are for witnesses or notary publics. Notary services are provided free of charge at the Curtis Laws Wilson Library on the campus of Missouri S&T, generally from 8am – 5pm, but you might like to call ahead to verify before you go. The library number is (573) 341-4008. You might also check with your local bank to see if they provide a notary service for their customers.
Many law offices also provide this service and can walk you through the process, if you would rather use them, and would have notary publics on staff.
