Phelps Health to hold community COVID-19 vaccine clinic tonight

Phelps Health will have Pfizer COVID-19 vaccines available on a walk-in basis at the Delbert Day Cancer Institute 5-7 p.m. Thursday, Sept. 30, for individuals age 12 and older. Any child below the age of 18 must be accompanied by a parent or legal guardian.
Pre-registration is not required, and there is no cost for vaccination. First, second and third (booster) doses will be available. Requirements for the booster shot recently changed – get details here.
If you are planning on attending this clinic, please complete and bring a consent form completed to ensure fastest service. If you will get your second or third dose at this clinic, please bring your vaccine card.
