COVID-19 vaccine clinics coming up

Phelps Health will hold appointment-only vaccine clinics on the following days:
- Friday, April 2, at the Rolla Church of Christ, located at 1303 Highway E. (Nagogami Road)
- Monday, April 5, and Tuesday, April 6, in the Phelps Health parking lot on 10th Street, located across from Phelps Health’s emergency department
The Pfizer vaccine will be administered. For an appointment, please register at covid19vaccine.phelpshealth.org/mychart/covid19#/. Individuals must be age 16 or older and in Phase 1 or Phase 2 of Missouri’s COVID-19 vaccination plan. Missouri S&T faculty and staff are included in Phase 2.
Please wear a face mask, a short-sleeved shirt and bring the following with you to your appointment:
- Your photo ID
- Your insurance card
- COVID-19 vaccination screening and consent form, available at https://bit.ly/3vwdyXT.
