Vehicle accident procedure

Accidents involving university-owned vehicles or vehicles rented or leased by or for the university for official business must be reported on UM Form 5. All university-owned vehicles have an envelope in the glove box with instructions on what to do in case of an accident. Report all vehicle accidents within 24 hours if injury or death is involved — otherwise, within 48 hours of the accident. Be sure to complete all boxes on UM Form 5, including the second page tab, and include the university vehicle license number, VIN number, MoCode and insurance information from the other party. Email the vehicle accident form as an attachment to, or send it via campus mail to Betty Birkner, 108 Campus Support Facility, or fax it to 573-341-6077.
