Free flu shots will be available to UM Choice Health Care Program members from 8 a.m.-4:30 p.m. Thursday, Nov. 13, in the human resource services conference room, Room 206 Centennial Hall (formerly University Center). Employees and dependents must show their Coventry ID cards in order to receive the free flu shot.

Employees not covered under Coventry can purchase a flu shot for $26.

The free flu shots are offered to Missouri S&T employees and their Coventry-insured spouses and dependents age 9 and older. Parents are required to accompany and sign a consent form for dependents ages 9 to 18. There is no requirement for a physician authorization.

Employees may print temporary ID Cards through their Coventry Health Care Online Member Services Account at www.ummedcvty.com. After logging in, select “Display your current ID card.” Pop-ups must be enabled to view your ID Card. You may print the temporary ID Card from this screen.
